In line with our Supporting Pupils with Medical Conditions Policy, we will only give your child prescribed (not over the counter) medicine, and only if you complete and return this form. If the period of administering medicine is eight days or more, we will discuss an Individual Healthcare Plan with you.
I understand that I must deliver the medicine personally to the school office. I accept this is a service that the school is not obliged to undertake. I understand that I must notify the school of any changes in writing. I consent to medical information concerning my child’s health to be shared with other school staff and/or health professionals to the extent necessary to safeguard his/her health and welfare. Each item of medicine must be delivered in its original container and must be clearly labelled with the following information: pupil’s name, name of medication, dosage, frequency of dosage, date of dispensing, storage requirements (if necessary) and expiry date.
